For and in consideration of my participation to appear as a model for any purpose which Rockstar, Inc. (“Rockstar”) may reasonably see fit, in the event such materials that are produced (via videos, photographs, audio tapes, and/or through any medium now or hereafter known) are used, sold, or exploited in any way, and in further consideration of the publicity to be received by the undersigned by the use of such materials, I hereby give Rockstar or its agents, representatives and assigns, those for whom it is acting and those acting with its permission or authority and its respective agents and employees and any person or entity who may hereafter publish the materials (hereinafter collectively referred to as the “Authorized Parties”), the absolute right and permission to copyright in its own name, use, reuse, publish, republish, exhibit, distribute, record, display, print and reprint in advertising material, television and motion pictures, magazines, books, video recordings, audio recordings and/or any other media, whether known or unknown at this time, and for any other purposes as they may see fit, in their sole and absolute discretion, those materials (hereinafter collectively referred to as the “Materials”). Such Materials may be used in any way by the Authorized Parties for purposes of trade, advertising, publicity or promotion of Rockstar’s goods and/or services, or any related goods/services, without restriction anywhere throughout the universe.  I acknowledge that I have not received, and will not receive, any compensation regarding my voluntary participation, nor will I receive any future compensation regarding the Materials.  I further acknowledge that any performance or contribution by me regarding the Materials shall be the property of Rockstar, and that I own the copyright in any materials submitted by me to Rockstar .
I hereby waive any rights to inspect or approve the Materials or the editorial or advertising copy or printed matter that may be used by the Authorized Parties in conjunction therewith, and I expressly waive any rights that I have arising out of or related to the Materials, known or unknown.  I waive any claims against Authorized Parties that I may have to the eventual use to which the Materials may be applied, known or unknown, including but not limited to any right of publicity and/or privacy claims.  The Authorized Parties may use the Materials and accurate or fictitious statements attributed to me which may constitute endorsement of services or products. 
This waiver shall be binding upon my agents and assigns.  I have read and fully agree to the terms of this release and acknowledge that I may be giving up considerable future legal rights against the released parties.  I agree to this release freely and voluntarily, without any promise or inducement by the released parties.  I hereby represent that I am of legal age (18 or older) and have read the authorization and release prior to clicking “Accept” below.
